Muco-Cutaneous Lymphnode Syndrome



Abstract
So-called Muco-Cutaneous Lymphnode Syndrome (MCLS) was reported by Dr. T. Kawasaki in 1967 firstly and then about 1,857 cases of this syndrome had been reported in Japan until 1971. The pathogenesis of this syndrome was not known at present time and on the other hand there was no report of this syndrome in other countries.
Authors had been interested and collectedd the cases like to this syndrome through past two years in this country. Here authors. presented the clinical datas of eight cases simulating this syndrome as reported by Kawasaki.
Five of eight cases were the infants at the age of one to two years and all of them was below four years old.
Seven of eight cases were admitted during summer (Table 1) season (June, July, August) and seven of them were male infants. All of cases had the episode of high fever and irritability which was persisted over one week, six of them showed the characteristic symptoms of cervical lymphnode swelling, sclero-conjunctival injection and papulo-macular erythematous rashes on the trunk and extremities.
Five of them manifested the swelling of depending erythematous swelling of hands and feet (Table 2), and the firely stained lips and oral mucous membranes.
The fever was continued for about 10 days, after then gross or fine desquamation of fingers and toes tips were appeared after the returning to normal temperature around the second week of illness.
The notable laboratory findings were leucocytosis, elevated ESR, positive CRP and normal ASTO titer (Table 7). On serum electrophoresis there was the tendency to increase alpha 1 and alpha 2 globulin.
This syndrome was very similar to classic type of scarlet fever, but there was some difference from this symptomes in the types of rashes and desquamation, negative throat and blood culture, and the seasonal occurence.
It seems, this may be an interesting entity of disease which might be evaluated by extensive broad field studies.
